Late Cancellation and No-Show Policy:

  • Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the therapists’ day that could have been filled by another patient or individuals on our waitlist. We require 24 hours notice for cancellations.
  • Late Cancellations made within 24 hours of your appointment are subject to 50% of the total cost of your appointment.
  • No-Shows (no communication received by the clinic via email, text, or phone call prior to your appointment) are subject to the full cost of your appointment.
  • These fees will be charged to your credit card on file and cannot be billed to insurance. Thank you for your understanding and cooperation.

First Visit & Insurance Disclaimer/Secondary Insurance Billing:

Please bring photo ID to your first visit to the clinic, thank you! Please contact us at info@evolvevancouver.com or 604-600-7399 to clarify if we offer direct billing services for your insurance. There may be unforeseen circumstances related to your insurance coverage where we must collect the full payment for your visit.

  • Telus eClaims does not allow secondary insurance direct billing. If your secondary plan falls under Telus eClaims, you will have to manually submit your secondary insurance. We will provide you with an insurance receipt to submit to your secondary Telus eClaims plan. See Telus eClaim insurance companies here.
